CTA's Community Connection Project (CCP) is a nonprofit, community-based adult (ages18+) transition program which focuses on providing supported, integrated, community experiences for individuals with significant communication and behavioral challenges with diagnoses such as autism. The vision of Community Connection Project is to bridge the gap between school and adulthood. To achieve this vision, CCP provides natural opportunities for our program participants to discover what they want to do in their adult lives and develop and practice skills to make these choices a reality. Their natural supports and chosen provider networks are then trained on what they want and what supports and plans are needed to be successful in those environments. We will help program participants feel empowered and to feel like an essential contributor to their own lives. CCP individually trains community members, support team and providers, vocational/volunteer venues, and any other adult networks that our program participants and their loved ones have chosen for them.
